Escape `~` as `~0` and `/` as `~1` when `flattenToPointers` appends object keys, while preserving caller-supplied prefixes. This keeps literal keys distinct from nested paths and makes flattened paths round-trip through state-store reads and writes. Add regressions for token escaping, nested prefixes, collisions, and store get/set round trips. Document the path behavior in the core README, core skill, and devtools guide.
