CODEX ACCOUNTS: choose one Codex account and show remaining quota

Summary

• One active Codex account is stored for every Codex model. The first imported account becomes active; later imports keep the current choice.
• Automatic switching is off by default. When it is on, an unavailable account is replaced only before the first response content, and only if another ready account exists. Manual mode reports the error and waits for the user.
• Account management shows the plan, remaining weekly and 5-hour quota with reset times, and the reset-card count with the nearest future expiry. Opening the dialog refreshes quotas. Buying a reset is not included.

Test plan

• Rust selection, stale-patch, and account-failure retry tests
• Codex plugin tests for usage windows and reset-card expiry
• Desktop account-list tests for countdowns and null metric deadlines
• In the account dialog: current account, automatic-switch explanation, cancel and confirm delete, quota lines, and reset-card details
• Grok and Antigravity add-account dialogs still open
