…note The default-branch support (#81) manages a github_branch resource, but the plan-summary namer had no case for it, so it rendered as "unknown type github_branch. resource address: ...". Add a case so it reads `github_branch :: <repo>/<branch>`, matching github_branch_default. Also fix docs/renaming-teams-and-members.md: since #74 named organisation resources, they no longer render as "unknown type github_team".

github_branch(managed since #81) had no case in the graformer plan-summary namer, so it showed asunknown type github_branch. resource address: …in PR comments. Add a case →github_branch :: <repo>/<branch>.Also fixes a stale line in
renaming-teams-and-members.md(org resources are named since #74, notunknown type).
