Adds the fork-meta documentation and tooling set:
- FORK_WORKFLOW.md: the patch-series spec and rebase pipeline
- agents/rebase-agent.md: agent instructions for driving a rebase
- .github/workflows/{fork-rebase,upstream-watch}.yml.disabled: optional
CI runners, kept out of the workflow glob via the .disabled suffix
