docs: remove hardcoded tables of contents from hand-written pages GitHub renders its own outline for wiki pages, so the hand-maintained "Table of contents" / "Index" blocks were pure duplication and drifted out of sync with the headings they listed. Removed 47 blocks across 47 hand-written pages (Platform documentation, LearnGAMLStepByStep, Recipes, Tutorials, WikiOnly and Community), covering the several shapes these blocks took: plain link lists, nested lists, blockquote-prefixed entries, self-referencing "(PageName#anchor)" links and an empty "## Content" stub in PredatorPrey.md. Auto-generated files are deliberately untouched: their tables of contents come from the doc generator (gama.documentation XSL), so removing them here would be undone on the next regeneration. Removing those needs a change in the generator instead. Also dropped the duplicated "# Documentation" title that the removed block had been separating from the page title. The change is deletions only (0 insertions); every removed line was a TOC heading, a same-page anchor link or a blank line. No page links, image references or heading anchors were affected.
